Initial Comments:
This report is the result of a State licensure survey conducted on July 11, 2023, at Hurwitz Center For Plastic Surgery, P.C.. It was determined the facility was not in compliance with the requirements of the Pennsylvania Department of Health's Rules and Regulations for Ambulatory Care Facilities, Annex A, Title 28, Part IV, Subparts A and F, Chapters 551-573, November 1999.











Plan of Correction:




51.4 (a) LICENSURE
Change in Ownership/Management

Name - Component - 00
51.4. Change in ownership; change in management.

(a) A health care facility shall notify the Department in writing at least 30 days prior to transfer
involving 5% or more stock or equity of the health care facility.


Observations:

The facility failed to notify the department of a change in ownership per 28 Pa. Code 51.4 (a).
Findings include:
On 3/7/23 during application renewal it was noted that a change in ownership occurred on 10/17/22. The department was not notified in writing 30 days prior to transfer involving 5% or more of the stock or equity of the health care facility.






Plan of Correction:

As soon as we were made aware of the deficiency the Facility corrected the deficiency by submitted for name change on 03/07/2023.

The Facility has new owner and all state policies have been reviewed and documented within the practice.

We received email notification 08/30/23 that the name was changed to Pittsburgh Center for Plastic Surgery

We are a Class A facility and were not aware of the 30-day policy. We did however notify AAASF which was our understanding to be the proper course of action.

Kathy Ottaviano has been named the employee to be responsible in monitoring the implementation of the plan of correction.

(b) A health care facility shall notify the Department in writing at least 30 days prior to a change in
ownership or a change in the form of ownership or name of the facility. A change in ownership shall mean any transfer of the controlling interest in a health care facility.


Observations:

The facility failed to notify the department of a change in ownership per 28 Pa. Code 51.4 (b).
